If an organization is not on the above list, we will not accept their voucher.

Owners are responsible for following up with organizations assistance. Owners must also make sure to communicate with organizations and ask if their voucher was emailed (messagesaah@gmail.com), faxed (909-883-1280), or mailed out to Arrowview Animal Hospital (766 East Highland Ave San Bernardino, 92404) before the day your voucher is needed.
Be advised that if your voucher is not in on the day of service you will be held responsible for payment.
If an organization is not listed, we are unable to accept its voucher. Please note that accepted organizations are subject to change without prior notice.
Pet owners are responsible for coordinating directly with the assisting organization and ensuring that all required estimates are provided. Owners must also confirm with the organization that the voucher has been sent to Arrowview Animal Hospital before the scheduled appointment date.

Arrowview Animal Hospital does not assist with application processes for organizational aid. If a voucher is not received on the day of service, the owner will be responsible for the full cost of services. No refunds will be issued for vouchers that are not presented at the time of service.
